REMINDER: The Non-Renewal Notice must be submitted prior to April 1st to be in compliance of the Participation Agreement. A form submitted after the April 1st deadline will not halt services in the proceeding fiscal year.
NOTE: Please ensure the form gets sent to the delegated signer for your institution. If you wish to cancel the services at the individual level, please contact your administrator. This form is designated to halt services at the Institution Level.
